YCS Registration Forms

If you are not registered, below, please find links to the forms required to register your children.  Open registration for the 2021-22 School Year starts on September 7th.

Families needing to register must email or call to schedule an appointment. Email us at mainoffice@yorkvillecommunityschool.org or call us at (212) 722-5240.

Please print each of the documents listed below, complete them and bring with you when you come to register.
